Before You Arrive

Pre-Admission Testing
A member of the hospital scheduling staff or your physician’s office will call you to schedule any necessary pre-operative testing ordered by your physician.

Depending on your diagnosis, these tests may include X-Rays, electrocardiograms (EKG), Ultrasound and/or blood testing. When you arrive at the hospital for testing, please report to the
Admitting Department located on the first floor adjacent to the entrance. Please bring all necessary documentation, medications you are taking, physician’s orders, your Med-Key card and all primary and secondary insurance cards, including your Medicare card and your advance directive.

Once you’ve been registered, the admitting nurse will directly assist you through each procedure and provide you with any necessary instructions regarding your surgery or procedure. This process usually takes between one and three hours, depending on the type and number of tests being performed.

Under most circumstances, your physician will coordinate with the hospital to schedule the date of your surgery or procedure. Please check in at the Admitting Department at your scheduled check-in time. If you did not undergo pre-admission testing and have not been pre-registered, please bring all necessary identification, including your Med-Key card, insurance cards and advance directive. If you are undergoing outpatient surgery, family members and friends may wait for you in our Surgical Waiting Room.

In order to expedite the admission process, we must have certain information at the time of registration, including:

• A picture I.D., such as a driver’s license
• A copy, front and back, of your current insurance card
• A completed claim form, if required by your insurance company
• Your full legal name
• Your social security number
• Your date of birth
• Correct demographic information, such as address, phone number, etc.
• Employment or retirement information
• Physician orders, if given to you
• Your advance directive, if you have one already prepared
• Medication List

What to Bring
You are welcome to bring personal care items such as a toothbrush, toothpaste and deodorant.
For your own protection, please do not bring jewelry or other valuables to the hospital, as we cannot be responsible for the safety of your valuable items. However, if you have any items
you would like to protect, they can be secured in our hospital safe. Please contact your nurse to make arrangements.
