Discharge

At Bartow Regional Medical Center discharge planning begins upon admission to ensure all patients have the safest possible discharge. A member of Social Services visits patients identified by Physicians, Nurses, and Case Managers to assess for post discharge needs. Then, Social Services assist them and their families by arranging necessary post discharge services.

When your attending physician writes the order for discharge, your nurse will review with you your discharge instructions regarding diet, activity and medications. If you have any valuables in the safe please remind the staff before discharge. After you return home, please be certain to follow the instructions given. If you have any questions, do not hesitate to
call your physician’s office.
