What puts me at risk for falls?
1. Being in different surroundings
2. Change in your medications
3. Medications such as diuretics or anti-hypertensives
4. Diminished vision
5. Prior bed rest
6. Frequent use of the bathroom
7. Feeling anxious
8. Being in pain
What will the staff do to help me avoid falling?
1. A Fall Risk Evaluation will be completed upon admission.
2. Reassessments will be completed daily.
3. Staff will check in on you frequently to provide toileting assistance and to meet other needs.
4. Keep your personal items within easy reach: telephone, bedside stand and water.
5. Ask the staff to check your footwear. Footwear should be non-skid slippers or slipper socks.
6. If you are a high risk for falling, a Fall Risk band will be placed on your arm, on the door, and on your chart. This will alert all staff that you need assistance with ambulation.

Here’s how you can help:
1. Communication with healthcare staff is very important.
2. Use your call light and wait for the staff to assist you in getting out of bed, or up from a chair.
3. Avoid leaning on the bedside tables or anything in the room that has wheels.
4. Take advantage of toileting when offered.
5. You may be tired and weaker than usual. Please use your call light for assistance.
6. Write down questions for your doctor.
7. Do not rely on family or friends to get you out of bed. Ask Bartow Regional Staff for assistance.
8. Tell the staff if you have a history of falls.
